FALL RISK SCREEN
Tell your provider about any/all falls; this should be assessed yearly.

DEPRESSION SCREEN
Depression is a serious illness that can affect how you think, feel and behave and can worsen health problems; this should be assessed yearly.

HEMOGLOBIN A1c
Diabetes can lead to complications such as kidney disease, nerve damage and eye problems. This lets your provider know how well your diabetes is controlled.

BLOOD PRESSURE
Elevated blood pressure can lead to heart attack, stroke and kidney disease.

BREAST CANCER SCREEN
Women 50-75 years old should have yearly mammograms.

COLORECTAL CANCER SCREEN
Men and women 50-75 years old should have either:
A colonoscopy every 10 years,
A Flexible sigmoidoscopy every 5 years,
A Cologuard, or stool DNA test every 3 years, or
A Hemoccult, or fecal occult blood test every year.

DIABETIC RETINAL EXAM
Damage to the retina of the eye can lead to loss of vision. Diabetic patients are especially prone to retina damage and should have this exam performed yearly.

SENSATION OF FEET
Nerve damage in the feet can cause problems with balance, as well as foot ulcers. Diabetic patients are especially prone.

KIDNEY FUNCTION TESTS
To assess kidney health and function; diabetic and hypertensive patients are more susceptible to develop kidney disease. This testing may be done by blood and/or urine.
